help please

I have a '95 teg gsr. has anybody ever expirence a timing belt that won't hold adjustment. Hear's what happens after adjusting the belt and starting my car it hold until I rev up about 4k rpm and it would start to squeal, I then turned off the engine and would see the belt loose and belt jumped a tooth or so. Any ideas?

I did install a new belt, I have new cam gears (I did not install cam gears thinking this would cure the belt loosing up) I did a engine build and the only items I did not replace was the water pump and the belt tensioner.
